This will really improve ability and functionality of a caregivers. I know the reasons I leave are because often the client goes into the hospital for a couple of weeks or so..and when that happens the aide is transferred to another client . They usually have choices if they want to stay…but sometimes they dont.
A streamlined evaluation (similar to the initial eval.) is done during each “maintenance” visit to identify any substantial changes on the patient’s needs that will require some adjustment.
